The yearly filling out the detailed history forms is ridiculous! Give each patient a printout of the file or have them view online and make changes. Do you realize how difficult it is to give dates and back track when you are coming to an appt directly from work, caring for elderly, kids, etc. Yes, it is not only this office, Image care is similar, but other practices can show you your surgeries, meds, etc and you can update as needed---NOT starting from scratch as if you are new patient.....Especially when you have been going to this office for over 15 years! Thank you.
